The Luxor Aus Made hotel cushion inserts list polyester fibre filling, memory resistant, in Twin Pack and Four Pack sizes from 30x50cm to 65x65cm, and in rounds from 35cm to 60cm.

For a ready-filled cushion. The Bedding House listings include the Chelsy 40cm x 40cm square cushion and the Blue Bird round cushion at 40cm diameter. KAAT Amsterdam lists square cushions at 45cm x 45cm and oblong cushions at 30cm x 70cm and 40cm x 60cm. PIP Studio lists neck roll cushions, Accessorize lists a 45cm x 45cm square cushion, and Bianca lists a lumbar oblong cushion at 30 x 60cm.

Is the cover included?

It depends on the listing. Titles with Filled include the filling. Most titles with Cover are the cover alone and most titles with Insert are the filling alone, but a few listings break that pattern, so read the listing.
